Services Offered
Assessment services can be customized based on the presenting needs and age of each client.
All assessments include: 1) a written report summarizing the evaluation process and results, as well as 2) a personalized Feedback Session via telehealth to verbally discuss the results.
Once Moxie Spectrum receives your interest, a free consultation appointment (via phone or video) will be scheduled to determine assessment needs, fit, and scheduling.
Telehealth assessment is not appropriate for everyone.

This service focuses on evaluating whether someone meets criteria for Autism Spectrum Disorder (ASD). This is an in-depth evaluation, not a screening. It includes a full IQ test, adaptive behavior composite, sensory intake, collateral reporters, social responsiveness data, records review, comprehensive diagnostic intake, and Autism-specific behavioral coding evaluations.

This service focuses on evaluating for ADHD/ADD. It is an in-depth cognitive assessment, not a screener. It includes a full IQ test and additional measures to evaluate executive functioning, working memory, and processing speed. It also includes collateral reporters, records review, and a diagnostic intake.

This service includes a full IQ test, plus additional testing for Specific Learning Disorders such as Dyslexia, Dyscalculia, and Dysgraphia. It also includes a brief diagnostic intake, records review, and collateral reporting.

This service combines aforementioned assessments and evaluates for a combination of ASD, ADHD, and/or SLDs as these can be frequently occurring comorbid diagnoses.

Assessments can be customized when necessary to fit your testing needs. For example, someone who had a recent IQ test may only desire portions of an aforementioned assessment protocol. Customization, and the corresponding change to the time and cost investment, will be discussed during your free consultation appointment.

ESA Evaluations are offered virtually. Pricing and duration of the assessment appointment varies, and will be customized based on whether this is an add-on service to another assessment or a stand-alone evaluation.

There are instances in which folks find themselves needing a “rush order” assessment. Such is often the case when someone needs testing accommodations for a standardized exam (e.g., GRE, LSAT, MCAT, etc.) with a specific firm deadline that may occur before the next available assessment appointment. When possible, time-sensitive assessments can be scheduled outside of Dr. Gouge’s normal working hours for an additional fee.

Folks may desire a variety of post-assessment support needs that are logistical, clinical, or administrative. Examples may include requesting:
a follow up Feedback Session
completion of an agency-specific accommodation form
completion of a disability questionnaire
verbal or written consultation with another healthcare provider, employer, or educator
These services require a completed Release of Information (ROI) form, and depending on the time needed to complete the task may incur an additional charge.
